Principle
Compare D-dimer with the YEARS threshold determined by three independently assessed clinical criteria.
Scope
Suspected acute PE in adults excluding pregnancy. Restricted to a clinically stable context and a highly sensitive quantitative FEU assay; do not use to delay management of instability. No age-adjusted threshold or pregnancy-adapted YEARS.
Interpretation limits
The entered value is compared before any rounding. The result describes the rule; it is not a standalone diagnosis.
Formula and logic
No YEARS criterion: D-dimer <1000 ng/mL FEU. At least one criterion: <500 ng/mL FEU. A value equal to the threshold does not meet the exclusion rule.
Interpretation
- YEARS exclusion threshold met
- The YEARS rule allows consideration of excluding PE without CT pulmonary angiography if its conditions of use are met. Verify context and assay method.
- PE not excluded by YEARS
- The YEARS threshold is not met: continue diagnostic assessment according to context. Elevated D-dimer does not confirm PE.
