Bronchiolitis · descriptive scores

Two descriptions of the respiratory examination, with distinct scales and populations. No automatic choice.

Wang / WBSS

Principle

Description of the respiratory examination; four contributions, total only.

Scope

Professional descriptive use before 24 months. Grid transposed from De Rose 2023, table 1; original Wang 1992 table not read, without certified equivalence to the original. The De Rose cohort was under 3 months: its performance is not generalized to all children under 24 months. No individual prognosis or disposition.

Interpretation limits

Descriptive total only, without severity class, individual prognosis, disposition or conversion to Tal/HAS. Apnea, cyanosis and exhaustion require assessment independently of the score.

Formula and logic

WBSS = respiratory rate + wheeze + retractions + general condition. Four contributions; total 0–12.

The added terms are point contributions under this scoring system, not raw measurements.

Modified Tal Golan–Tripto2018

Principle

Description of the respiratory examination; four contributions, total only, room-air SpO₂.

Scope

Modified Tal Golan–Tripto2018, before 12 months. Study of 50 hospitalized infants; descriptive use in emergency or outpatient care is a context extension not validated by this study. Study exclusions: previous hospitalization for wheezing, chromosomal abnormality, chronic cardiac or pulmonary disease. No other Tal, severity class or disposition. Withdrawn former tool not reactivated.

Interpretation limits

Descriptive total only: no severity class, disposition or conversion to Wang/HAS. Apnea, cyanosis and exhaustion require independent assessment. Never remove oxygen to obtain a score.

Formula and logic

Tal Golan–Tripto2018 = age-dependent respiratory rate + room-air SpO₂ + wheeze/crackles + accessory respiratory muscles. Four contributions; total 0–12.

The added terms are point contributions under this scoring system, not raw measurements.
