4AT — delirium screening

Delirium screening; not-testable items score according to the 4AT.

4AT — delirium screening

Principle

Screening for delirium and cognitive impairment.

Scope

Local French adaptation of 4AT v1.2: abbreviated labels and contextual guidance. Adult screening, particularly older people. Consider communication and usual functioning. NT can score 2 for AMT4/attention, unlike Glasgow. Unknown information is not a negative response. Validation evidence for the official instrument does not validate this adapted interface.

Interpretation limits

≥4: possible delirium, with or without cognitive impairment. 1–3: possible cognitive impairment. 0 does not exclude these disorders. Missing data block the total, but an already supported flag ≥4 remains visible; diagnosis requires clinical assessment.

Formula and logic

4AT = alertness(0/4) + AMT4(0/1/2) + attention(0/1/2) + acute change(0/4). Total 0–12.

Interpretation

0–0
No test flag
1–3
Possible cognitive impairment
4–12
Possible delirium
