Classic SOFA — Organ scoring
Principle
Sum of explicitly entered contributions; no default clinical value.
Scope
Adult in critical care. Six supplied clinical ratings, each 0–4, corresponding to six organ maxima over the same 24 h window. Sum 0–24; original variant distinct from SOFA-2.
Interpretation limits
Transcription of clinical organ ratings, not automatic classification of raw measurements or sepsis diagnosis. Use each organ's worst rating within the same 24 h window and the highest level met. Rounded 1996 laboratory intervals and their gaps are not reinterpreted automatically. An undetermined rating remains missing; no total if a domain is missing. No mortality percentage, treatment threshold, estimation under sedation or normal-value imputation.

SOFA-2 — Organ scoring
Principle
Sum of explicitly entered contributions; no default clinical value.
Scope
Adult in critical care. Six supplied SOFA-2 clinical ratings, each 0–4, corresponding to organ maxima over the same 24 h window; sum 0–24. 2025 model distinct from classic SOFA.
Interpretation limits
Manual clinical scoring of organ maxima within the same 24 h window; no automatic calculation of ratios, drug equivalents or RRT indications. Apply support and alternatives in Table 2 of SOFA-2. An undetermined rating remains missing, without a final total: the pilot does not impute missing observations to 0 or carry forward the previous day's observations, unlike the study rules. Insufficient for sepsis diagnosis and supplies neither individual mortality nor a treatment indication. Do not confuse SOFA-2 thresholds with classic SOFA.
