Principle
Quantify the observed neurological deficit. Administer in the prescribed order, promptly, without coaching except where allowed. Score observed performance and the first attempt as specified by each item; a software correction fixes transcription, not a better second performance.

Adult NIHSS, standardised neurological examination by a trained assessor. Fifteen components, distinct left/right sides. NT/UN or a missing component: no total, without imputation. Insufficient to exclude stroke or choose thrombolysis/thrombectomy. Abbreviated local translation; use standard NINDS instructions and materials, not reproduced here.
