HEART
Principle
Risk stratification of major cardiac events following chest pain in the emergency department.
Scope
Adult version, 2013 validation scoring system. First conventional troponin; no HEART Pathway, serial troponin protocol or automatic transposition to high-sensitivity assays. A score does not delay ACS management, particularly with ST elevation.
Interpretation limits
Stratification using the 2013 variant, without individual probability or discharge/admission decision. The score alone neither confirms nor excludes ACS; no HEART Pathway.
Formula and logic
HEART = H + E + A + R + T; each component scores 0, 1 or 2. Total 0–10.

- 0–3
- Low cardiac event risk
- 4–6
- Intermediate cardiac event risk
- 7–10
- High cardiac event risk
