ALGOPLUS reference threshold ≥ 2/5: probable acute pain, management requires clinical judgement. A lower score does not exclude it; not a chronic pain scale. Score each domain once, without subtracting behaviour for its presumed cause. No treatment step, medication or dose proposed. Variation does not prove treatment efficacy. Locally reworded labels, not the official grid.
